The Easter Seals mission is to create solutions that improve the lives of children and adults with disabilities or other special needs and their families. In October 2001, Easter Seals New Hampshire convened a group to discuss the plight of frail seniors, many of whom have Alzheimer's disease or a related disorder. This led to the Seniors Count initiative. Seniors Count raises the visibility of the frail elderly in New Hampshire, generates regional solutions and helps leverage public and private resources. The goal of Seniors Count is to create long-lasting, systemic change in the design and delivery of services to the frail elderly, allowing them to remain safely in the community for as long as possible. Seniors Count is a collaborative effort that draws on resources and expertise from throughout the community to meet the critical needs of frail and vulnerable elderly people.

ServiceLink is a free information and supported referral service for older adults, adults with disabilities or chronic illness, their caregivers and anyone interested in long-term care planning. With local offices in 13 communities and satellite offices statewide, ServiceLink can answer questions and help make connections to local services that support healthy and independent living. The NH Department of Health and Human Services, Bureau of Elderly and Adult Services support ServiceLink in partnership with local non-profit organizations.
